The diet and training programs provided by Shokunbi Solutions LLC/DrHalidFit are not and will not be, reviewed or approved by any fitness professional or licensed physician. Therefore, by accepting these terms, you do not hold Halidu Shokunbi or Shokunbi Solutions LLC/DrHalidFit responsible for any injury, illness, allergic reaction, or lack of results while using a program from Shokunbi Solutions LLC/DrHalidFit or at any time in the future. You completely acknowledge that you are simply receiving advice and it is your choice to adhere to the provided advice. You understand Halidu Shokunbi and Shokunbi Solutions LLC/DrHalidFit are NOT a certified personal trainer, nutritionist, dietician, physician or any other licensed/certified professional in any field besides physical therapy. Your participation in this program is your own responsibility in its entirety and everything has been disclosed to you in the following waiver.

I hereby affirm that I am in good physical condition and do not suffer from any mental or physical disability which would prevent or limit my participation in a program provided by Shokunbi Solutions LLC/DrHalidFit. I understand that I am participating in a program of strenuous physical activity including but not limited to aerobics, weight training, cardiovascular exercise, and other aerobic and anaerobic conditioning routines and exercises, ect.

I understand and am aware that strength, flexibility, aerobic and anaerobic exercise including the use of exercise equipment, and any DrHalidFit program may involve me in are potentially hazardous activities. I also understand that fitness activities involve a risk of injury and even death, and that I am voluntarily participating in these activities. I also understand the use of exercise equipment and machines is dangerous and I accept all responsibility while using this equipment. I hold myself responsible for any injury caused by improper technique, use of exercise equipment or negligence by Shokunbi Solutions LLC/DrHalidFit  or myself. While using a DrHalidFit program I am responsible for any and all injuries and any other possible health or non health related issue that may arise. I hereby agree to expressly assume and accept any and all risk of injury, illness and death.

In consideration of my participation in a program from Shokunbi Solutions LLC/DrHalidFit I, for myself, my heirs and assigns, hereby release Shokunbi Solutions LLC/DrHalidFit, its owner and employees, independent contractors, and officers from claims, demands, and any causes of action arising from my participation in diet or training program from DrHalidFit. I fully understand that I may injure myself as a result of my participation in any diet training program from Shokunbi Solutions LLC/DrHalidFit, and hereby release Shokunbi Solutions LLC/DrHalidFit from any liability now or in the future including but not limited to heart attacks, muscle strains, broken bones, shin splints, back injuries, allergic reaction or soreness, occurring during or after my participation in the diet or training program.

Any supplementation advice provided by Halidu Shokunbi on behalf of Shokunbi Solutions LLC/DrHalidFit is strictly done so by opinion only. Any companies or products mentioned with Shokunbi Solutions LLC/DrHalidFit are not affiliated with the site and are not liable for any negative repercussions. By agreeing to these terms you are accepting that you will not hold Halidu Shokunbi or Shokunbi Solutions LLC/DrHalidFit accountable for any issues, health related or non-health related, that may result from taking a product. You are responsible for understanding your own body and the health risks involved, as these products have not been approved by the FDA.

This agreement constitutes the sole and entire agreement between Shokunbi Solutions LLC/DrHalidFit, Halidu Shokunbi, and myself in regards to the diet and training programs described above. No other agreement exists between us, and Shokunbi Solutions LLC/DrHalidFit has made no representations to me, verbally or in writing, about such training program except as stated herein.
